Case Manager The Case Manager provides intensive case management, assessment, counseling, advocacy, stabilization planning for families in St.
Mary's Center's adult family shelter programs.
Work Schedule:
Monday - Friday 9AM - 5PM ( One Late Day 10AM - 6PM or 11AM - 7PM) Pay/Salary:
$47-53k +5k Bonus OR $49k-55k Responsibilites:
Case Manager works collaboratively with Housing Specialists, Behavioral Health Clinicians, and Family Partners to develop and maintain professional goal-oriented relationships with assigned families in shelter Case Manager works closely with families beginning at intake to understand families' individual needs and acts as their advocate and support system through their length of stay in St.
Mary's Center's shelter programs Case Manager will work collaboratively with the team to provide a trauma informed environment, helping families achieve their goals and actively work towards their rehousing and stabilization plans Case Manager will serve as a positive role model and help advocate internally and externally for families' needs.
Case Manager will maintain accurate Case Management files and reporting records for internal and external systems as required by the Contractors, Licensors, and St.
Mary's Center's policies Case Manager will provide crisis-intervention with families in collaboration with the Behavioral Health team and leadership Requirements:
Bachlor's Degree required in realted field 3 year's of Case Management experience On-Call schedule for one weekend per month and one day per week.
Fully vaccinated against COVID-19 at the time of hire.
Must be able to lift and/or move up to 25 pounds Stand for extended periods.
