RN Care Manager

The Primary Care Behavioral Health (PCBH) program is co-located within the Primary Care Clinic. PCBH Mental Health (MH) professionals complete brief treatments for a variety of MH problems, and/or triage Veterans to specialty MH clinics. The General Mental Health Clinic provides assessment and outpatient treatment for Veterans with a range of mental health problems, with an emphasis on mood and anxiety disorders. Treatment is geared toward reducing psychiatric symptomatology and patient distress, strengthening skills and coping resources, and improving psychosocial functioning and quality of life. The incoming RN Care Manager will work within an interdisciplinary, supportive team environment. The RN Care Manger position is integrated within both the PCBH and the outpatient GMH program. He/she is expected to contribute to the growth of these outpatient MH programs by providing care management, consultation, education and collaborative care functions for Veterans with mental health conditions, e.g., mood and anxiety difficulties, sequelae from post-traumatic stress, problematic substance use, stress management, grief, lifestyle behaviors such as poor diet, smoking and low exercise that contribute to physical health issues such as chronic pain, insomnia, obesity and diabetes. Care management is guided by evidence-based nurse-delivered telephonic models to provide monitoring of adherence to treatment, outcomes and medication side effects, patient education and motivational interventions, and assistance in referral to specialty MH care programs. While the majority of patient contact is via telephone and initiated by consults from PCBH and GMH staff, face-to-face visits are within the RN's scope of practice. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Experience:
Prior Psychiatric/Mental Health nursing experience, training and experience in integrated behavioral health care and registry-based systems for tracking patients and clinical outcomes A strong background in the provision of brief behavioral interventions using evidence-based techniques, e.g., Motivational Interviewing, Behavioral Activation, Problem Solving Therapy Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
