Behavioral Health Care Coordinator - LCSW

GENERAL SUMMARY/ OVERVIEW STATEMENT:
The Behavioral Health Care Coordinator will serve as an integral member of a multidisciplinary team for Psychiatry and the Population Health programs providing assessment, intervention and advocacy as needed to patients with varying levels of mental health conditions. This position requires a high degree of flexibility, independence, and knowledge of the health care system and community resources to maximize emotional, social and physical well being of the patients being served by the MGH Primary Care Practices.
PRINCIPAL DUTIES AND
Responsibilities:
Screening/Assessment
Triage consultation requests from Primary Care Physicians and forward them to the appropriate Behavioral Health
provider, providing timely turnaround of information to primary care practices.
Coordination/Communication
Employing a range of interventions, such as care coordination/case management, risk screening and behavior change, interventions, information, or referral and safety planning.
Research resources available for adults with behavioral health needs, including but not limited to: outpatient individual orgroup therapy, psychopharmacology, partial hospital programs and inpatient programs, and self-management tools.
Effectively track and document calls, case reviews and outcomes for reporting purposes.
Identify and refer to community services as indicated by consultation or communication with patient.
Communicate effectively with community-based mental health providers to find the most accessible
resources for patients, considering coverage, geography, and availability.
Advocate on behalf of patients and their families to gain access to services and resources.
Assist the interdisciplinary team with follow up of patient calls as recommended during consultation either for BH eConsults or Collaborative Care for coordination of care needs
Provide backup for answering Behavioral Health eConsult calls or e-mails as needed
Attend weekly case reviews as assigned
Organize meetings with assigned group of BHSS team members on a monthly basis to provide guidance, additional support and/or education on relevant issues related to care
Organize weekly 1:1 meetings with BHSS from assigned practices to review case load capacity and program documentation standards
Provide support with complex cases for Behavioral Health Support Specialists at assigned practices as needed
Meet with Program Manager monthly on key program dashboard measures such as program utilization and capacity as well as any barriers or concerns
Document all relevant patient contact information in the patient's electronic medical record
Collaborate with the local and central behavioral health integration teams (including Medical Director and Program Manager) to identify areas of opportunity to improve integrated behavioral health management and achieve quality goals
Participate in practice outreach and educational presentations, as needed
Complete other projects and duties, as needed and specified by BHI Team and/or supervisor

SKILLS & COMPETENCIES REQUIRED:
Knowledge, skills and ability to assess patient need for services.
Knowledge of community agencies/resources.
Knowledge of specific medical/psychiatric illnesses, procedures and treatments, related medications (i.e. psychotropic), acuity versus chronicity
Comfort with a patient-centered approach that allows the patient's desires around behavioral change to inform the plan of care.
Ability to advocate/negotiate systems for/with patients.
Ability to maintain effective working relationships with patients/families and staff.
Excellent interpersonal and written and verbal communication skills
Ability to work well in interdisciplinary teams including health care providers and office staff, representatives from Primary Care, Psychiatry, Social Work, and Population Health Management and to build collaborative relationships
Ability to work independently with minimal supervision

Maintains patient/family confidentiality
LICENSES, CERTIFICATIONS, and/or REGISTRATIONS:
LCSW required.
EDUCATION:
Master's degree required.
Field of Study/Additional Specialized Training:
Degree in Social Work or other related field
Experience:
Candidates must demonstrate ability to understand the role of social worker in a hospital setting and to work in a complex, fast-paced environment.
Must have demonstrated ability to work with medical professionals and individuals experiencing mental health challenges and their families.
Must have knowledge of community resources, crisis intervention skills, and an ability to work with a multidisciplinary team.
WORKING CONDITIONS:
The social worker will mainly work out of the Outpatient Department of Psychiatry but must have the ability to travel to primary care practices to which s/he is assigned.
Must have ability to travel independently to Primary Care locations in, Boston, Waltham, Chelsea, Revere, Everett, Charlestown.
